No. We do not replace physicians, specialists, or therapists. Instead, we act as a bridge—making sure clients understand their care plans, follow through with appointments, and that providers are updated on progress or concerns.
The frequency depends on your needs. Some clients require weekly visits or calls, while others may only need monthly check-ins. We create a schedule that provides the right balance of support while respecting independence.
Yes. One of our core goals is to reduce unnecessary ER visits and hospitalizations. We do this by monitoring symptoms, managing medications, providing education, and ensuring timely follow-up after hospital discharge.
Yes. We guide clients and families through Medicare, Medicaid, Managed Care Organizations (MCOs), Family Care, and local Wisconsin resources. Our role is to connect you to the most appropriate services while advocating for your best interests.
Our rates vary depending on the level of service (hourly, flat fee, or package-based). We provide a transparent financial agreement before services begin. While insurance may not cover all services, many families find the investment saves money by avoiding costly hospitalizations and unnecessary care.
